Referral
The patient is referred into the TCC service, either as an existing inpatient using a direct EMR referral, or as an out patient directly from home.
Onboarding
The clinical team onboards the patient to the TCC phone app, configures the correct peripherals for their care needs and educates the patient on their use.
Monitoring
The patient is monitored on a daily basis. Using the TCC App, the patient answers questions on their condition while the relevant bio-metrics are gathered from their peripherals (BP, Weight, Pulse Oximetry etc.)
Management
Should the patients health deteriorate, the clinical team monitoring them can intervene early. This can take the form of medication adjustments, in person review or referral to other specialities.
Support
Using the TCC App, patients can get ongoing support, targeted education and always remain in contact with their care team, while in the comfort of their own home.
